Booking Form and Terms & Conditions

 

Booking Forms can be obtained from Reception or you can download a hard copy by Clicking Here. Alternatively you can fill in the form below and it will automatically be sent to us electronically.

 

Please ensure that you have read our Terms and Conditions. If you need  to read them again click on the menu to the left of the screen, or to download a copy  - Click Here. They can also be found attached to the Booking Forms available from Reception.

 

NOTE - Please telephone first to ensure that the room is available before completing any booking form. We have had forms sent to us when the room requested has already been taken by another group.

 

All sections must be completed in full. The manual forms should then be returned to Reception at the Centre. All bookings must fall within the limits of our lease which state that the Centre should be used for Health, Education and Community Activities. We are not allowed to be used for political or religious activities. Parties are limited to ALL children attending must be under eight years of age and must be held at the weekend. Should the Trustees feel that your hire is not acceptable, for any reason, you will be contacted as soon as possible after receipt of your form. When you request use of our equipment, you may be asked to complete an addition form to ensure that what you require is available. Please be reminded that it is the hirers responsibility to put up and take down any tables and move chairs to the required positions and equipment, crockery etc. from other rooms should not be removed.
