Booking Form and Terms & Conditions

 

Booking Forms can be obtained from Reception or you can download a hard copy by Clicking Here. Alternatively you can fill in the form below and it will automatically be sent electronically to us.

 

Please ensure that you have read our Terms and Conditions. If you need  to read them again click on the menu to the left of the screen, or to download a copy  - Click Here. They can also be found attached to the Booking Forms available from Reception or you can complete the form below, which will send a copy to the Centre Management Team.

All sections must be completed in full. The form should then be returned to Reception at the Centre. After approval by the Trustees, you will be contacted to confirm that your booking is acceptable and available. All bookings must fall within the limits of our lease which state that the Centre should be used for Health, Education and Community Activities. We are not allowed to be used for political or religious activities. Parties are limited to ALL children attending must be under eight years of age and must be held at the weekend.
